For the user attributes Password Policies require, you can use any existing attributes that are not currently in use.  You will have to check the docs, but I believe the disabled flag is of type String and the others are Binary.

Go to the properties of the User Directory and verify you have configured the user attributes that get used for Password Policies (password attribute, password data, disabled flag, etc).

I believe that for Apache on Windows, it doesn't automatically detect the path to your web server.  While running the Web Agent Configuration Wizard, select the option that allows you to manually enter the full path to the web server root.
