Are the file permissions on the file set so that IIS has permissions to display it and IIS is set to allow anonymous access? If you disable the agent in the WebAgent.conf does the error still occur?

Do you have the ISAPI role/feature enabled? Does the web agent handler have execute permissions? Does the user running in the app pool have permissions to the samples and logs directory? The user may also need permission to log on…
